Outreach
Case Management Program
Our Outreach program is a heartfelt response to the challenges faced by unhoused individuals in areas where significant resources are often allocated to policing. LEAD (Law Enforcement Assisted Diversion)
Case Management Program
LEAD, our Law Enforcement Assisted Diversion initiative, represents a transformative, community-based approach to responding to low-level criminal offenses. We recognize the impact of unmet behavioral health needs and suspected violations, and instead of perpetuating a cycle of incarceration, we divert individuals to a trauma-informed intensive case management program. This program, focused on harm reduction, provides a range of support services, including housing and substance use disorder treatment, showcasing our dedication to addressing the root causes of issues faced by underserved populations.
Coordinated Entry
Case Management Program
Our Coordinated Entry program is a proactive effort to identify individuals in need and establish a scaled, appropriate response. Through the diligent work of our Housing Navigators, we assess and prioritize chronically homeless individuals. By assisting them in gathering the necessary documentation for referrals to permanent housing, we exemplify our commitment to a compassionate, systematic approach that ensures that everyone, regardless of their circumstances, receives the support and care they deserve.
